If you have concerns regarding health/safety issues about a branch or program and would like to file a complaint or grievance, we will be more than happy to assist by having you complete the above. Please be aware that all information including your name will be kept confidential. The Branch Executive Director or the grievance hotline staff will contact you within five (5) working days after receiving the grievance.
As an alternative, you may write and mail a letter to the supervisor of the program or the Executive Director of the branch or, if this is a grievance, you may call the grievance hotline at (808) 531-3558, ext. 5888.
